It's ment to simulate the heeee heeeee hoooooooooooooo labor breathing
Use your space bar to make her breathe out. Depending on how well the user does results in a stamina loss mutiplier that may or may not help :P 0.5 to 1.5 being the caps, default being 1.25 if she doesn't do her breathing at all.

Naturally it'll pause when she's pushing.
